- Notice to Financial Institution Establish IOLTA Account (To be completed by the attorney when setting up a new IOLTA account)
- Notice of Closed IOLTA Account (To be completed by the attorney when an IOLTA account is closed)
- IOLTA Compliance Statement (To be completed annually and returned to the Clerk of the Supreme Court prior to March 1. Also, to be competed by the attorney whenever his/her status changes during the year and mailed to the Arkansas IOLTA Foundation)
- IOLTA Post Notice (To be posted in the lawyer's office)
Forms for Banks:
- Attorney Trust Account Overdraft Reporting Agreement (To be completed by bank as a prerequisite to participation in IOLTA program)
- Confirmation of IOLTA Account (To be completed by the bank for each new IOLTA account established)
- Error Report Form (To be completed by bank when a remittance error occurs)
